CS Washing your hands amongst many other mistakes

    Advertisements

Just took CS in Houston. MAde a lot of mistakes.
Does anyone know how much forgetting to wash your hands hurts you? I forgot to wash my hands or use gloves on the first two cases. Also, I forgot to drape or wash my hands on the first case. The drape was over in the corner and I guess I didn't see it. Someone told me that is the worst mistake you can make on these cases. I don't know. Also, I completely whiffed on the backend of the history on one case. I had this depressed stick in the mud patient who was he!! to get an HPI out of. I got the 10 minute warning and flew into the physical exam. I forgot to ask about smoke, Med hx. etc. etc. Also, there were at least a couple of rectals I forgot to tell the patient about although I wrote in the note. Also, I started a couple of P.E. maneuvers without. asking the patient first. I also didn't finish one case and my otoscope piece got stuck in the patient's ear. I just kinda chuckled and said sorry.Overall, I felt my HPI's were very strong. My DDx and Procedures were acceptable. My Physical exam was midling. I was as disgustingly nice as I could possibly be and my face still hurts from all the smiling. Can anyone who has passed or failed this thing give me some feedback. I don't know what I'm going to do with myself unti Jan. 30th. Making someone wait this long to know if they have to sit out a year or not is inhumane.

"my otoscope piece got stuck in the patient's ear"...

Ok.. sorry but that made me laugh out loud SOOOooo obviously, you went beyond expected.. I don't think anyone (or atleast that I know of) have ever used the otoscope in CS... lol..

The washing part for the first two cases is bad but not horrible. the drapping part is kinda bad... BUT if everything else went ok (ie. notes) I'm sure you did fine. remember, they take the best ten out of twelve... so take away the first bloopers and you did fine...

I hope you passed. When you get your passing score you can recall the otoscope scene and laugh it off... great story to tell when you become an attending

    Good Luck

There is something terribly wrong with this exam. I have read many posts where people discuss the mistakes they made and still passed. I ofcourse made mistakes but nothing that should've failed me. I never forgot to wash/wear gloves, I greeted every patient and smiled, I draped every patient, reviewed the case with the patient, counseled when appropriate and performed lung and cardiac exam on every patient. I failed. I am in total disbelief. I wish there was some committe I could talk to. I can't possible imagine how I failed that exam. It's so mezmorizing that I can't help but think there is some sort of mistake.

    bigeasy.....where did you take it?

There are two parts to the exam. The integrated clinical encounter(which is data gathering + patient note). And there is the other part which is basically your rapport skills and your speaking profeciency. You have to pass BOTH parts to pass the test. So they can flunk you for bad bedside manor. That part of the test is subjective.

    Hey there.

I took it in Houston. Yeah, I know there are parts to the exam. I failed the data gathering and patient note....impossible!. I used the PAMHUGSFOSS pneumonic for every patient, I have years and years of clinical experience and I know my bedside manners are great. I received an A in every core rotation and my notes were well written. This is why Im so baffled. I'm even from Houston, talk like them...blah,blah. So I just can't see where I possibly went wrong. It's a shame not to get any feedback. It haunts me because in front of me was a student whose English was absolutely terrible and he had zero personal skills. Hell I felt sorry for this guy as he was FOB. I had to help him get around the hotel his english was so bad. I can't help but wonder if they somehow confused the two of us. That's how silly and helpless I feel about these results. It just doesn't make sense especially based on what I have read in other posts about peoples mistakes.

    hey thebigeasy,

Do you have any idea what could have gone wrong? Could it have been your patient notes? Or maybe you didn't ask enought questions to the SP about the CC or HPI? Any thoughts?
