    Many mistakes and still passed Step 2cs

    I sat step 2cs in May at the Chicago test center and I was pretty sure that I had failed. I knew that I had made a lot of mistakes, and when I read posts in which people were worried because of only a few small mistakes it only compounded my fears. However, I recently found out that I had passes and I am posting the following to show that there is hope even for people like me who made a lot of mistakes on the CS exam.

    My mistakes were as follows:

    I did not counsel a single patient about smoking, alcohol use, drug use or safe sex.

    I ran out of time in two cases and had to walk out half way through the physical exam in both.

    I forgot to ask about diet, drug use and weight loss in two cases where this information was important.

    I forgot to extend the leg rest in one case where this was particularly important.

    I forgot to tie up the patient's gown after the physical exams in most cases.

    I took my exam recently. I feel the same way, a lot of mistakes. It all went by so fast, I don't know what to think. I thought maybe I could have taken longer in the patient room, as i finished each case early (3-5min).

    Knock on door..."Are you Mr/Mrs XXXX"?..Hello, I'm Dr. XXXX.
    Take drape off stool and immediately place it on Pt's lap.
    " How can I help you today"?
    blah,blah,blah. Gather enough info for you HPI.
    Perfrom "PAMHUGSFOSS" questions.
    Put on gloves/wash hands....do chest,lungs and focused exam.
    " Here are some things I believe might be causing this problem.."
    These are some tests I will run to help substantiate the cause.
    Do you have any quetions for me? I would also like to take the time to ask you if you have thought about quitting smoking/drinking/drugs/safe sex (if applicable).
    Ok. Let me summarize what you've told me to ensure I haven't missed anything you want me to know.
    Give brief summary.
    Any questions?....Have a good day.

    Now this is really all you need to do. Don't let anyone try and tell you it's any more difficult than that. The problems some face is that although people try and use culture as an excuse to their behavior..it simply isn't acceptable. For example..

    A quiet speaking, shy person is not a humble intelligent Dr, it's an insecure one.

    Not making eye contact is not polite, it's rude and impersonal.

    Interrupting a patient is not taking control, it's disrespectful.

    SO!!!! Speak up!! Look your patients in the eye and however difficult it is..........SMILE!!

    Those of you who are not familiar with American culture better brush up. You want to practice in the US, then you have to learn how to conduct yourselves appropriately.
