    A 25yo woman with MVP with mitral regurgitation is scheduled for tonsillectomy.2yr ago she experienced hives and wheezing after receiving penicillin for strep pharyngitis.
    What’s the most appropriate antibiotic regimen for bacterial endocarditis prophylaxis?

    A:amoxi 2gr orally 1hr before the procedure
    B:ampi 2gr either IV or IM 30min before procedure
    C:clindamycin 600mg orally 1hr before the procedure
    D:clindamycin 600mg IV 30min before the procedure
    E:no prophylaxis is necessary

    D:clindamycin 600mg IV 30min before the procedure

    pt at moderate risk!ab prophylaxis is necessary,ampi contraindicated because she’s allergic to penicillin and IV clinda the best choice because the pt should be NPO before surgery.
