    a 8 y.o. boy comes in to your clinic with complaints of pain in the right forearm, he states that he was climbing a tree when he slipped and fell, he is is great pain and cannot move his arm. an x-ray is taken and there is a fracture to the radius. his arm is placed in a cast and he is sent home. 3 hours later he is rushed to the e.r because he is in severe pain, and he is having difficulty with passive extension of his wrist? 1.what is the diagnosis? 2. what do you do next?

    The patient has a radial fracture which has led to compartment syndrome. Compartment syndrome is an increased pressure in a cavity which leads to comprimise of the blood supply to the muscle , soft tissue and nerves. The signs and symptoms of COMPARTMENT syndrome are remembered by the 4 p's 1.Pain on Passive Motion (out of Proportion with Phyical exam) 2.Pulselessness 3. Paralysis 4. Paresthesia .Also remeber to watch out for Volkmanns contracture with any radial fracture. Oh yeah and also Compartment syndrome is most commonly occurring in the ant compartment of the arm and leg, Diagnose by physical exam and you can also check the Delta pressure (I think, which is the End diastolic pressure - compartment pressure) and treat with immediate fasciotomy. Thats all I can remeber now, and I apologize if anyof this is wrong, please coreect me, its just off the top of my head...good luck to all
