Go to the on-campus clinic the day of the exam. Make sure to mention to the doctor that you have an exam that day and he'll fill out a form. When you get back from the clinic send an email to the course director explaining the situation. You don't need to include details (e.g. symptoms).

A business day or two later you'll get an email from the Dean of Students office saying that they received your medical excuse. Then someone will be in touch with when you need to take the completion exam. For a final exam it is at the start of the next term.

It appears the school policy has changed somewhat and the course director can chose to give a cumulative or essay completion exam, presumably to scare of people who fake illness to get additional study time.

the same thing people who pretend to be sick...go to the clinic, get a note, and then do a makeup...be warned though, the makeup is usually an essay exam...NOT something i would suggest having to do...

i have taken a few make ups for being sick and they were the same format as the exam my friends took the day of the exam. it wouldn't surprise me if i took the same exact exam as them. the profs might say that to scare people but they're too lazy to change the exam and also they can't curve your exam properly if you're the only one taking that exam. what the school does tell you is that if you're not feeling well don't take the exam.

When I took the anatomy completion exam it was the same exam as given at the end of term. The coversheet had the final exam date in December on it.

The term 3 midterm makeup was really diffrent from the original. There were short answer and essay questions. That department has a lot more time on it's hands though.

I'd avoid make-up exams at all costs. I'd only skip the exam if I was on my death bed.

Anatomy make-ups apparently don't give you answer choices for the practical - you have to write in the answer; on the other hand, you're allowed to jump back and forth between tables during the allotted 25 minutes, instead of having only exactly 1 minute per question

I can verify this is 100% correct. They just let us loose in the room with 25 minutes to go.

Assuming that the final practical was like the midterm, there is also no CTs, MRIs or X-Rays. I can't say for certain since, well, I clearly missed the final.
