I graduated from this univ. 10 years ago. It is just an average russian medical school. I do not know anything special about orthopedy program. I know they have lots of experienced doctors and you should probably have a good practice there. But as far as I know, there is some shortage in the available study matherials. The medical library is very poor and internet acces is not available everywhere. I would suggest you purchase a books and your own study matherials, and bring it with you. Most of the university faculties are not proficient in spoken english. They often using translators during the lections and seminars, but the translators are not very good either. So, you should understand - you have to study a lot on you own. But othervise it is a good place - lots of patients, lots of practice.
Ryazan is a big city, and the life is not very expensive. Much less expensive than in Moscow. But the Moscow is less then 200 km away - 3 hour on the train. You can live without a car - the public transportation is not bad. But I should also say that Ryazan it is not very safe city and crime rate is relatively high. Not too dangerous - just like all russian cities now. But anyway, be careful at night.
