    Programs starting PGY-2

    I'm an IMG - well almost - and there's one thing I don't quite understand about the programs that start as PGY-2
    my question is:
    if I apply to a 3-year program that requires a preliminary/transitional year does that mean that if I match they'll sign me up for the NEXT year and I'll have to match for a second program to do the PGY-1?
    or are those programs only for people who are already IN their PGY-1 ?
    Could someone please explain?

    You have to apply and match into a PGY-1 program too.

    ok so do i have to apply to a separate preliminary/transitional program?
    and does that mean that i match into 2 programs at the same time??

    and what happens if i don't match to PGY-1 - do i loose a year and have to do everything all over again?
    i'm confused...

    i'm confused...
    for example if u were entering the 2006 match for i duinno anesthesiology
    you would be applying for anesthesia spots starting pgy2 july 2007
    AND you would also be applying for prelim/trans pgy1 spots starting july 2006.


    u would have to apply to both advanced anesthesia programs AND you would at the same time apply to prelime/trans spots..

    during interview season u would be interviewing for two different things... and u would be matching into 2 diff programs at the same time.. one for 2006 for 1 yr and one for 2007 for 3 yrs..

    if on match day u dont match for a pgy1 position u will be informed that u didnt match for a pgy1 spot but u have matched for an advanced spot..
    in that case u can scramble into a pgy1 spot..
    tons of prelim spots avail through the scramble.. and u can set up ur pgy1 that way so that u wont end up wasting a yr...

    i think i got it now
    it's kinda like a double match right?
    one has nothing to do with the other

    i was just asking cuz the NRMP doesn't explain that on their website
    and i was wondering how is it possible to match into 2 different programs at the same time... since they always emphasize that you can only match into one...

    yeah its kinda like a double match.. what the nrmp is stressing is u cannot and obviously will not be matched for more than one place for the same time period.. for example..

    ill continue to anes as an example but this holds true for any speciality that requires u find ur own pgy1 (intern) yr...

    if u apply to some anesthesiology spots that are 4 yrs (have the prelim yr built in) and you apply to some anes. spots that are three yrs (and u have to find your own first yr)....then in that case u would still interview at prelim/trans places because u will need them for the 3 yr programs...

    BUT if u match at one of the 4 yr anes programs..
    then theres no way that u will match at any of the prelim/trans programs at the same time (because in that case for 2006 ud be starting a prelim spot somewhere AND at the same time in 2006 u would be starting a 4 yr anes spot.. and u obviously cannot be at two places at once..)


    nrmp achieves this by the way u set up ur rank list...
    u have ur primary rank list where u list all the anes programs..
    and then the programs that are 3 yrs and require a separate pgy1 yr.. u have to fill out a "supplemental" rank list.... or in other words a secondary rank list.. that second rank lists contains ur prelim programs..
    the second rank list will come into effect IF u match at the 3 yr program..
    and for each 3 yr program u can use the same secondary rank list or u can make unique supplemental lists for each 3 yr program based on location or whatver..

    the supplemental rank list basically means "if i match at 3 yr program XYZ, then this is where i would like to complete my first yr, from my highest preference to lowest"

    for the 4yr programs u will not be allowed to fill out a supplemental list...

    oh, thanks for a thorough explanation
    now i know it all

    well almost because i got one more q:
    what if for example i want to apply to lets say anest and as a backup i wanna apply to a few transitional or preliminary so i can give it another try the following year if i don't get into anest
    can i put the same programs on a main ROL and the suplemental one?
    just curious...

    yeah lets say for whatever reason (ie anes competitive) and u only get a couple(example 4) anes interviews..
    well u can rank those 4.. and then u can rank the remaining spots (5+) with ur transitional/prelim programs (they will be ranked in ur supplemental list and in ur actual primary rol)

    by doing that (i think its been discussed on another thread) if u have NOT matched in an anes spot.. u still have the chance of matching in a prelim/trans spot.. and u can scramble into an anes spot.. or u can look for a anes spot that has come avail during ur intern yr...


    this way wont waste a yr... and u will have ur prelime yr outta the way...

    ur ROL list would look like this

    1. Univ of xyz- Anes (4 yr)
    2. Medical school of abc- Anes (3 yr)

    ---------------------- Supplemental List 1
    ---------------------- 1) Univ of there - Transitional
    ---------------------- 2) Univ of here - Prelim IM

    3. PQR School of Med-Anes(4 yr)
    4. Univ of 123-Anes (3 yr)


    ---------------------- Supplemental List 2
    ---------------------- 1) Univ of here- Prelim IM
    ---------------------- 2) Univ of there- Transitional

    5. Univ of here-prelim IM
    6. Univ of there - Transitional
