    Rheumatic fever Vs. Post-strep GN

    Heh, guys i have a few questions Rheumatic heart and Post-strep GN are both caused by same organism group A Beta hemolytic strep. I know that it's at the autoimmune rxn to M-protein that actually causing the problem in both heart valve and glomeruli BM. In Rheumatic heart, it's HSN type II while PSGN is HSN III immune complex react with M-12 protein.
    Here is my question: 1. How come u dont get both rheumatic heart valve problem and PSGN at the same time even though it's same group of organism? It's a different strain that causes different manifestation? or it something else? 2. What is there a M-protein to Rheumatic heart dis? Thanks for all ur input.

    Post Strep Diseases

    Same bug causes both diseases. You are right on the HSN types in both diseases. The C carbohydrate of streptococcus differentiaties what Lancefield group they are classified as. Within the beta-hemolytic group A strep (strep pyogenes), the M protein differentiaties which strep group A will cause rheaumatic fever or post-streptococcal GN. Thats why u cant have both at the same time. Also, most cases of RF are found after pharyngitis adn PSGN is usually found after cellulitis or any skin infection with strep A. Antibodies cross react with M antigens in the heart. In addition, titers for ASO (anti-streptolysin) increase with RF, while anti-dnase titers increase with PSGN. In PSGN, immune complexes are deposited, activatign C5a. Hope this helps. Cheers.
