    canfused about renal?

    Advertisements

okay sorry guys its something really basic that i just dont understand.
can someone tell me what does sterile pyuria mean and what do you see in the urine?
there is a connection with positve leukocyte esterase + dipstick, caused by chlamydia trachomatis.
but you also see neutrophils? so there is an infection.
and no sign of bacteria? someone wanna explain it in a easier way?

white blood cells or white blood cell casts in the urine with a culture negative result

bacteria are there so you will get a positive stick but if i remember correctly you don't routinely test for GC using a urine sample and the media used to grow the usual suspects may prevent GC from growing? but i don't remember the details. if you suspect something other than the usual things you request to grow culture for that atypical organism or in the case of GC just swab it and get a regular GC test.

also wbcs aren't just a sign of infection they are also there for any inflammation like nephritis, lupus etc. come to think of it, TB is also not usually grown but can be tested for in this case.

double check all the above, its been a long time since i went over this stuff

    ...

a lot of things can cause sterile pyuria...
basically you have an elevated number of white cells, but per
culture techniques you can't grow anything...
example --> UTI that was treated with abx, you still might see
elevated whites, but no (+) culture anymore....
another one is taking excessive analgesics use causing some renal
problems elevating your white count (and not necessarily caused by
any organism)....you see this with kidney stones also...

I saw this question in another forum so I will also ask:
Why dont they prescribe Zytromax(azytromycin) for both gonorrhea and chlamydia? Goljan says think cheap.

I have nothing agaisnt the place but with the years living in the states I havent visited yeat.
