    From the bottom of my heart of hearts I am rooting for every one of you. It has been painful for me to read the board today. I know all of you are trying your best. I wish that each and every one becomes a GREAT doc, that I would go to or send my family to. I have enjoyed getting to know all of you, even those that I have sometimes had to be hard with or not quite gotten your sense of humor, but I think of ALL of you with good wishes and I really do mean that. Thats why this end of semester and all the changes I have brought are so hard for me because I see how hard they are for some of you, many of you in fact. Some of you post without reading the posts I have placed before, which answer many of the vents and frustrations that you are dealing with. I am going to spend some time going through as many as I can think of and if I forget any just PM me and I'll answer them. You all know you can reach me anytime at 660 8199 or 469 9087 and I am more than happy to listen or answer questions. Try not to call to late though UNLESS YOU NEED TO!

    1. Attendance- I know many of you don't listen in class well because of attention issues, or the professor's accent, or whatever you don't learn in an auditory fashion. I am giving my usual lectures at LSU during the break and I will get about 75% attendance. Sometimes the class size is almost zero. I get a bit more because they like my lectures. THERE IS NO ATTENDANCE POLICY IN MOST SCHOOLS IN THE US, IF IT WAS UP TO ME I WOULD DO AWAY WITH ATTENDANCE ALLTOGETHER, BUT ITS NOT UP TO ME. CARIB SCHOOLS HAVE AN ACCM ATTENDANCE REQUIREMENT AND MANY STATES NOW FOR LICENSURE HAVE AN ATTENDANCE REQUIREMENT FOR IMG'S THAT IS VERY CLOSE TO OUR 80% RULE THERE IS NOTHING I CAN DO ABOUT IT...I FEEL FOR THOSE OF YOU THAT SIT THERE AND DON'T LEARN EFFECTIVELY AND CAN'T STUDY IN CLASS. I WISH I COULD CHANGE THIS BUT I CAN'T.

    2. SHELFS

    A. AT 5% AFTER CAREFUL THOUGHTFUL DISCUSSION IT WAS CLEAR THAT MOST STUDENTS DIDN'T STUDY FOR THE SHELVES. THE SHELVES ARE THE MOST IMPORTANT TEST YOU WILL TAKE, ITS WHAT THE US STUDENTS USE TO GAUGE THEMSELVES, NOT THE EXAMS THE PROFS GIVE. YOU NEED TO PASS THOSE ALSO BUT THE SHELVES TELL THE TALE.
    10% VS 15% WITH THE END OF SEMESTER BEING 40% OF THE GRADE....WELL THATS THE WAY IT IS IN MOST SCHOOLS, AND THE REASON IS BECAUSE IF YOU HAVE BEEN STUDYING AND READING THE TEXTS AND LEARNING ALL ALONG, CRAMMING SHOULDNT BE NECESSARY, IF YOU ARE CRAMMING THEN YOU HAVE NOT LEARNED THE MATERIAL IN THE WAY THAT YOU WILL BE ABLE TO REFER TO IT 5 AND 10 YEARS LATER, THE WAY YOU WILL NEED TO TO UNDERSTAND ADVANCES IN MEDICINE AND NEW THERAPIES. YOU HAVE TO BUILD A TRUE INFRASTRUCTURE AS I SAY OVER AND OVER AND THAT DOESN'T MEAN CRAMMING, IT MEANS LEARNING THE MATERIAL, TRULY LEARNING SO IT STICKS WITH YOU FOR LIFE...SURE YOU MAY BRUSH UP FROM TIME TO TIME BUT THAT IS WHY MEDICINE IS LIFELONG LEARNING. SO THE FACT THAT YOU ONLY HAVE A DAY OR A WEEK TO STUDY IS NOT ACCURATE YOU HAVE THE ENTIRE SEMESTER TO LEARN THE MATERIAL. EVERY NIGHT IS A BLOCK NIGHT AND EVERY DAY IS A BLOCK DAY. READ THE BOOKS 40 PAGES A DAY COVER TO COVER AND TAKE NOTES!! THE PROGRAM IS ACCELERATED 20 MONTHS OF CONTINUOUS LEARNING, FOR MANY OF YOU WHO TRULY UNDERSTAND THIS YOU USE THE BREAKS TO REVIEW THE OLD AND GET A HEAD START ON THE NEW. I HAVE THOUGHT THAT PERHAPS IT IS BEST TO ONLY GIVE QUIZZES DURING THE SEMESTER, SURPRISE QUIZZES AND USE THE SHELF AS THE FINAL BUT THE PROBLEM WITH THAT IS THAT IT TAKES A WEEK TO GET SCORES BACK AND WE WOULD WASTE A WEEK OF LEARNING. IF WE DON'T HAVE THE GRADES BY THE END OF THE SEMESTER WE CAN'T PROPERLY DO NEXT SEMESTERS REGISTRATION SO THATS WHY THE SHELF THE WEEK BEFORE AND THEN THE FINAL......AGAIN MY HANDS ARE TIED BY PROFESSORS WANTING TO TEST NEW MATERIAL WHICH IS ESSENTIAL SO AS TO NOT WASTE ANY TIME IN THIS ACCELERATED PROGRAM AND BY THE DICTEM OF ACADEMIC FREEDOM.....TWO STUDENTS SUGGESTED THAT WE GIVE THE SHELVES THE FIRST DAY OF THE NEXT SEMESTER WHICH I THOUGHT WAS BRILLIANT AND WOULD GIVE YOU THE BREAKS TO STUDY, BUT THE ADMIN AND THE PROFS AND THE REGISTRAR NIXED THAT IDEA FOR ALL SORTS OF TECHNICAL REASONS, INCLUDING THE FACT THAT GRADES WOULDNT BE DONE AND IF WE LETS SAY, USED THE PHYSIO SHELF TO COUNT TO THE MICRO GRADE OF THE NEXT SEMESTER THAT WOULD BE UNFAIR TO THE MICRO PROFFESOR (FOR THE RECORD I STILL LIKE THAT IDEA BUT IT WAS NIXED ABOVE ME). IT ALL BOILS DOWN TO EVERY DAY IS A BLOCK DAY AND EVERY NIGHT IS A BLOCK NIGHT AND THAT IF YOU REALLY LEARN THE MATERIAL ALL SEMESTER WITHOUT PARTIES, TRIPS, ATHLETICS ALL SACRIFICED FOR THE SAKE OF LEARNING AND GETTING HOME SOONER THAN ALL YOU WOULD ONLY NEED AT THE END OF THE SEMESTER IS TO REVIEW YOUR NOTES. I KNOW YOU ALL NEED SOME MENTALHEALTH DOWN TIME BUT IT HAS TO BE THE MINIMUM NOT THE MAXIMUM.

    B. 3 SHELFS ARE TOUGH BUT SO IS MEDICAL SCHOOL. BECOMING A DOC IS MOTIVATION, DEDICATION AND COGNITIVE ABILITY. NOT EVERYONE WHO MEETS OUR STANDARDS WHICH ARE STRINGENT FOR ACCEPTANCE, WHEN THE RUBBER MEETS THE ROAD HAVE WHAT IT TAKES TO PASS. NOT EVERYONE WHO WANTS TO BE A DOC CAN BE A DOC. IT PAINS ME TO SAY IT BUT ITS A TRUTH. WE CANNOT JUST LET EVERYONE PASS A SOCIAL PROMOTION BECAUSE I LIKE YOU OR YOU ARE A NICE DESERVING PERSON OR EVEN THOUGH YOU WOULD BE A CARING DOC YOU HAVE TO MASTER THE MATERIAL AND THEN HAVE ENOUGH "STUFF" TO PASS EXAMS. OVER AND OVER ON THIS BOARD AND OTHERS AND THE US BOARD PEOPLE SAY "YOU MAKE YOUR OWN MEDICAL SCHOOL EXOERIENCE" AND THATS TRUE ESPECIALLY FOR THE US KIDS WHO NEVER GO TO CLASS. I AM TRYING TO RECRUIT NEW PROFESSORS AND KEEP THE BEST WE HAVE TO MAKE A DREAM TEAM AND I AM CLOSE, BUT THAT WILL NOT FIX THE PROBLEM FOR THOSE OF YOU WHO CAN'T MEET THE NEEDS OF AN ACCELERATED PROGRAM. YOU CAN ALWAYS SLOW DOWN TAKE LESS COURSES AT A TIME, I KNOW THAT COSTS MORE IN LIVING EXPENSES AND TAKES LONGER BUT FOR SOME OF YOU ESPECIALLY THE ONES WHO HAVE 3 SHELF ISSUES IT MAY BE THE BEST WAY AND THE ONLY WAY THAT YOU GET THROUGH. AGAIN IT PAINS ME TO SAY IT BUT ITS TRUE. IF I HAD EVERY SINGLE ONE OF THE BEST PROFESSORS I HAVE EVER MET IN MY LIFE HERE, THERE WOULD BE A LESS THAN 5% IMPROVEMENT OVERALL...ITS NOT ABOUT THE PROFS ITS ABOUT YOUR ABILITY TO MAKE YOURSELVES A DOCTOR THATS WHY THERE IS NO ATTENDANCE REQUIREMENT AT THE BEST SCHOOLS. THEY LEARN ON THEIR OWN AND USE OFFICE HOURS FOR TOUGH PROBLEMS.

    C. SHELVES BEING UNFAIR AND QUESTIONS REUSED OR NON REPRESENTATIVE. I HAVE HAD EVERY PROFESSOR TAKE THE SHELVES WITH SHEETS TO SEND BACK COMPLAINTS AND SUGGESTIONS TO THE NBME AND TO MAKE SURE THEY KNEW THE MATERIAL ON THE SHELVES, INCLUDING THE PHARM SHELF. THE PHARM PROFESSOR DID NOT SEND ANY HUGE COMPLAINTS BACK TO THE NBME, JUST A FEW QUESTION CORRECTIONS. THE TIME FOR HIM TO SPEAK UP IN MY HUMBLE OPINION WAS BEFORE THE TEST WAS GIVEN NOT AFTER THE EXAM IS A DONE DEAL. I HAVE TAKEN MOST OF THE SHELVES MYSELF, I TOOK THE COMP SHELF, I TOOK THE KAPLAN THIS WEEK AND I TAKE ALMOST EVERY EXAM GIVEN ON CAMPUS MYSELF. THE QUESTIONS ARE ALL SUPPOSED TO VIGNETTES AND I AM LOOKING TO REPLACE PROFS WHO DON'T TEST IN THE STYLE I WANT. I CAN'T FORCE THEM TO (ACADEMIC FREEDOM) BUT I CAN REPLACE THEM AND I RECRUIT AND LOOK FOR NEW PROFS ALL THE TIME. I MYSELF MAKE NEW QUESTIONS EVERY TIME I TEST AND I SPECIFICALLY TEACH WHAT I AM GOING TO TEST ON. DR F TAUGHT ME READ CHAPTER 10 TEACH CHAPTER 10 TEST CHAPTER 10 ITS NOT HARD AND ANY PROFESSOR THAT CAN'T OR WON'T DO THIS WILL BHE CHANGED. FORTUNATELY MOST DO.

    I WISH THERE WAS SOME MAGIC WAY I COULD MAKE IT EASIER FOR ALL OF YOU I TRULY DO FROM THE BOTTOM OF MY HEART OF HEARTS. BUT DR L AND I ARE NOT DUMB GUYS AND ALL THE SUGGESTIONS THAT PEOPLE HAVE MADE OF HAVE BEEN THOUGHT OF AND THERE IS A REASON. A LEGITIMATE REASON WE HAVE NO CHOICE.

    A CARIB MEDICAL SCHOOL IS A TOUGH ROAD. AT MUA WE ARE HERE TO GET 100% THROUGH. WE DON'T TAKE HUGE CLASSES KNOWING THAT A LARGE NUMBER WILL NOT MAKE IT. STILL MANY DON'T AND THE REASON IS AS SAID IT'S A TOUGH ROAD.

    I KNOW YOU ARE ALL MENTALLY AND PHYSICALLY EXHAUSTED. ANY OF YOU THAT ARE PARTICULARLY STRESSED SHOULD CALL ME OR DR L IF NOTHING ELSE JUST TO TALK TO US AND LET US BE SYMPATHETIC,

    I AM SO SADDENED TO SEE ALL THE FRUSTRATION. BUT IMAGINE THE FRUSTRATION I FEEL WHEN I HEAR PEOPLE HAVE CHEATED,OR PEOPLE COME TO ME WITH SELF SERVING REASONS EXCUSES WHY THEY SHOULD PASS WHEN THEY HAVE NOT EVEN READ ONE PAGE OF THE TEXT ALL SEMESTER AFTER I PLEAD WITH THEM TO, OR PEOPLE DON'T HEAR ALL THE REASONS I WANT TO DO THE THINGS TO THE CURRICULUM THEY WANT, BUT FOR REGULATORY REASONS I CAN'T AND THEY DON'T LISTEN TO ME.

    IF NOTHING ELSE YOU SHOULD GET FROM THE BOARD THAT I AM TRYING TO DO THE BEST I CAN FOR ALL OF YOU FROM DEEP WITHIN MY SOUL WORKING 16 HOURS 7 DAYS A WEEK TO KEEP UP WITH EVERYTHING. I MAKE CALLS FOR YOU TO CLERKSHIPS, I WILL GIVE ANYONE EXTRA HELP WHO ASKS, I KEEP THE ROOMS OPEN TO STUDY (NO FOOD..THATS A JOKE) AND I WILL KEEP TRYING BECAUSE I LOVE MEDICINE, IT IS A RELIGION TO ME, I LOVE THE SCHOOL, I ENJOY YOU FOLKS AND I LOVE THE ISLANDAND MOST OF ALL I LOVE TEACHING AND THE JOB OF BEING DEAN.

    NEXT SEMESTER THE HOSPITAL EXPERIENCE IS GOING TO BE EVEN EXPANDED!

    THE SHELVES WILL BE CURVED I AM ALMOST CERTAIN BUT CAN'T PROMISE (ACADEMIC FREEDOM) BUT IF I AM ASKED FOR MY OPINION I WILL TRY TO GET THEM CURVED CORRECTLY,

    I AM GOING TO CURVE THE COMP SHELF, AND THE KAPLAN WILL BE ADJUSTED BECAUSE THE NEW TEST IS JUST TO DARN HARD. I HAVE HAD COMMUNICATION FROM KAPLAN AND WE ARE WORKING THROUGH THE WEEKEND TO UNDERSTAND HOW HARD THE NEW TEST REALLY IS. I TOOK IT AND IT'S HARD.

    I GUESS I COULD HAVE PUT THE END FIRST BUT THEN NONE OF YOU WOULD HAVE READ THE INSPIRATION IN BETWEEN! DEAN T

    I'm not arguing for a change in the way the shelfs are carried out, but I understand other people's frustrations. I also like the idea of have the shelfs after the semester break. Perhaps a way of not making the shelf for the previous semester count towards the grades of a current class would be to not have the shelfs count at all towards a particular course, but be counted on their own. For example the three shelfs given for Med 1 could be weighted together as a 1 credit course and the grade go on the transcripts. They would be become part of the grade-point average, so it would definitely be taken seriously. (Not that we would get another credit--it would just be the weight of the grades for the purpose of the GPA.) Classes would just have their final exams and class grades could be done by the end of the semester for administrative purposes. This could accomplish the main goals of using the shelf as an assessment tool that also carries enough weight as an incentive to do well. And, since the shelf exams wouldn't be tied to a class it could be given at a time that allows students to prepare properly for them. After all, the comprehensive shelf for Med 5 doesn't become part of the grade for any particular class, does it? Anyway, I'm not pushing it, just trying to think outside the box.

    I'm sorry. What I said didn't seem like an idea already considered. If I was being redundant, then excuse me. As I said, I'm not overly frustrated and accept the scheduling as a given and will work my best under the circumstances.

    My 2 Cents

    I have simple solutions to some of these issues.

    ATTENDANCE:
    If there really is an 80% requirement for licensure, and I am skeptical that there is, why not make it 80% across the board? The fact that some profs require 90 and 95% attendance is ridiculous. Attendance policy should be mandated by administration, period.

    SHELF EXAMS:
    Why not order the shelf exams at the beginning of the course, and let the professors 'teach to the test'? The professors could simply incorporate shelf material into the existing curriculum - eg, "these drugs were d/c 10 years ago, but you still need to know them for the shelf"

    The Attendance Rule Is In The Accm And In Several State Board Licensing Requirements. Stop By My Office And I'll Show you.

    Academic Freedom Forbids Admin From Setting The Attendance Requirements I Have Tried To Get Dr D And Dr A To Go To 80% When The Accm Comes They Ask Who Dictates Attendance

    Shelfs To Be Reviewed Must Be Requested And A Lengthy Contract Signed That I Am Happy To Share With You.

    The Only One I Didn't Think Of Was Brought To Me By 2 Med 2 Students Who Suggested Giving The Shelves The Following Semester And Counting Them Towards Next Semesters Grade I Thought That Was Brilliant But Admin Nixed

    On an "up" note, i really appreciate that the cumulative shelf and kaplan are being reviewed by the dean. I like that staff wrote the post-test themselves. As a current med 5 student, who reads the texts, studies constantly, and is genuinely trying as hard as I can, I appreciate that my efforts will not be met with futility.

    Thank you, Dean T, I hope that something is worked out for this. As someone who is a good student, I can tell you that I am still genuinely apprehensive about the post due to the potential detail that could be asked on such a vast amount of material. I feel confident I will know it well to write the USMLE within 3 months, but this Kaplan test has me rattled.

    As for the comprehensive, if I had even 2 more weeks, I think I could have aced it...but once again time was of the essence.
