AUA and KMCIC are two different schools, that are in the same family.
If you apply and attend AUA, you do your basic sciences in Antigua. If you apply and attend KMCIC, you do your basic sciences in Manipal.
Both schools do their clinical rotations at the same sites after basic sciences. Meaning, you will be doing rotations with AUA and KMCIC students.
Each school has a separate application -- meaning you must apply (and pay to do so) to each school, if you want to be considered for admission at both.
