If you can get into a school Stateside, do it. Even if it is D.O.

Krakow is a modern medium sized city and comes with everything that entails. There are parts of the city I wouldn't go to alone, just as in any other. I've been here on holiday several times and never had a problem. I have some friends of Indian descent living in Krakow and they've never had a problem to my knowledge. They tell me that people there are quite cultured.

It would be a good idea to ask Jag directly about the fees. They have emails and phone numbers on their website.

The school is liberal in the sense that there are a ton of scientists and the teachers are quite used to dealing with American students, but Poland is a religious country. I wouldn't fear religious persecution if I were you, though. The school is not liberal in the sense that it IS a medical school and therefore there are certain proprieties that must be followed. Medicine and Tradition are intertwined in Poland (as in other European countries and in India, I suspect) so don't expect to TP a building. How well you fit in is entirely up to you. I have a few Indian friends and they are some of the most chill people I know.

Make a good impression early for rotations. I hear it's not that hard. Some of the upper years might be able to answer this better.

I don't know how to answer your last two questions. You will get out of it what you put in. If you want a medical education, you will get it, but medicine is mostly self-taught. Didactic lectures are the vestigial remnant of an era when access to knowledge was limited. I'd urge you to reconsider attending an American Medical school. Some of them aren't as hard to get into as you might think. Especially in the South.
