What to follow in Rx: Kaplan or UW?

    Advertisements

I have encountered several treatment options in USMLE world that do not coincide with what Kaplan notes say. What should I follow? Kaplan or USMLE world?

For example: In USMLE world a pxn with acute pneumoccocal meningitis, the empiric treatment is Ceftriaxone+ Vancomycin (it states that pneumoccocus has become resistant so it should be given empirically) + Ampicillin (if elderly)
However in Kaplan its just Ceftriaxone and give Vancomycin only when there is documented resistance or at high risk of resistance + Ampi if elderly and post neuro surg.

I have encountered others like this. What should we follow?

Why dont you read "Mubarak's Principles of Medicine", its good and concise. IT tells you exactly what to do with a patient of pneumococcal meningitis!!

I have encountered similar problems too. UW says Crytococcal meningitis in AIDS patients should be treated with Amphotericin and Flucytosine followed by lifelong Fluconazole. But Kaplan says only Amphotericin, then followed with Fluconazole... no mention of Flucytosine.
Then again, in Endocrinology... what's the appropiate treatment modality for Graves Disease? Is it Radioactine Iodine ablation alone? or Antithyroid meds, and beta-blocker to reduce the symptoms?

i just took exam yesterday so i think i have good input. UW and kaplan are both good for individual reasons. uw is a great teaching tool, but question phrasing SUCKS. Kaplan has excellent phrasing, but not as difficult as the real thing. dont worry about ambigous tx they wont test u on that. u need a solid foundation in all of medicine and other specialties. Its not this first or that second or anything like that. u need general principles on how to confirm a dx, or how to do test to rule out diseases. pointer: focus on medicine and look up any answers u are not comfortable with, and i gaurantee u it will come out on exam. The tend to test things that arent covered on 'review books' so dont just stick to one book. pick 3 and reread twice, do questions inbetween and this helps immensely. gl
