    DM type II

    q)A DM type II pt is under glipizide. His current glucose level is 270. what is the next:
    1.add insulin
    2.add metformin
    3.add pioglitazone
    4.add glyburide





























    Answer is add metformin

    Isn't metformin considered first-line?

    Also, consider what the condition was when the blood was taken. Was he fasting? Did he just have a coke? You shouldn't make decisions based on just one reading. The only thing that has been really shown to be a predictor of morbidity and mortality is the A1C level. I typically make my decisions based on that. You can also look at serial readings and get an idea of what is going on.

    i would pick metformin because eventhough pioglitazone would work, it has higher side effects...

    Yep metformin they have the same question in UW under IM Endo section. However, before starting Metformin make sure u check the pt renal status (Serum Crea level), because there is lactic acidosis with metformin. So u might want to look at it factor if they give u someone with renal problem.
