Previous post is total crap. J1 lasts for 7 yrs. Up till then you can do residency/fellowship training before waiver job/returning to country of origin for 2 yrs.
yes you are correct. However J1 visa is for one specific job only. You cannot change it to another category.So basically you cannot change to fellowship program.

Do some home work. Just ask any J1 resident/fellow at your hospital.
Bottom line, you have to be smart, pro-active and hard working to get ahead in America, NOT be American. That's what this country is founded on.
However, there are still many hurdles for visa holders though. My hospital just stopped accepting visa's this yr for my IM program.
DocSAM is full of crap unfortunately. little_indian_boy is correct. Do the hard work and you will get what you want. I am a Canadian on a J1 Visa and I just secured a fellowship. It is totally doable. I have numerous colleagues who have done the same.
