    I got this in my inbox today, and i have no idea what the school is referring to. Would anyone like to elaborate before I get highly annoyed and write the sender back an email that will probably consist of me telling her that I was never informed of the NYSED - and then probably getting more annoyed with her responses which will probably be highly condescending?

    Please and thank you, email as follows:

    Dear student.


    A serious issue has been brought to my attention. Given instructions to you prior to Clinical rotations regarding NYSED application and fee for Long and Short Term clerkships in NY, NYS either has not received your application/fee payment or has not sent me/you your acceptance letter.(I have no copy on record of being sent)

    BY LAW, you were not and will not be permitted to rotate in the state of NY unless they have received this application and payment. If you have rotated in the state of NY and this application and payment was not received, your accreditation for prior rotations could be compromised.

    If you have not sent your application and payment, please do so IMMEDIATELY. Application and Payment is to be sent DIRECTLY TO THE NYSED. I will require a copy of the application so that I am aware that you have complied.

    This is a serious issue and must be addressed immediately.

    If you have any further questions, please feel free to contact me.

    I googled it and the closest thing I could find was this:
    NYS Medicine Application Forms

    which states AUA does NOT need to fill out the application because we are approved in NYS. All of the forms in the link are also for AFTER graduation...so I'm not really sure. Let us know if you find out anything else!

    Every time I read the email, I want to write back a very annoyed response telling her I have no idea what she's talking about, and that if it were SO urgent, I should have been informed earlier (maybe when on the phone with my CA the hundreds of times I talked to her) in the past year and a half I've been in clinical rotations. A search of my entire gmail inbox (since 2005) shows nothing regarding the NYSED - I also looked at the very first clinical handbook I got and NOTHING.

    WHAT GIVES AUA! GET YOUR ACT TOGETHER!

    Also, it would have been nice if we were told our clinical advisor had changed. You CALL me the second my loan checks are due, but URGENT matters like this, and the fact that the person I've been speaking to regarding rotations is now gone requires no phone calls.

    ...even more annoying.

    ok after a quick search of the forum, apparently newer students were told to do this -- not sure why they were told and not the older students.

    I found this site: NYS Medicine Application Forms
    nothing on there applies to me does it? I'm not fifth pathway, I never did any cleckships in another country... i'm so lost.

    Someone also sent me this gem from the very organized ROSS site (take note AUA)

    I was emailed such a form with my initial clinical stuff. It has to be filled out if you are doing more than 12 weeks of rotations in NY. It is a very basic pdf file that you just write your name on and mail back to the NY State Education Board with a $20 check.

    Yeah you need to fill out this form. I've been pushing AUA to mail this form to all students for 4 years and they've told me it was unnecessary and their lawyers have already looked into it. And, it looks like they've made a sudden change in policy.

    There's no penalty for filling out the form late. Just include a little note of apology and they won't care. Schools like St. Georges automatically mail out this form to their students before starting clinicals.

    So you fill out one form and check the long-term box before doing your in network rotations in NY, and you fill out a separate form for each away rotation that you do and you check off the short-term box on those applications.

    NYS will delay up to 3 months after you send in the application, because I remember there was a huge procedure and they required a lot of documents - I forget which exactly and I remember that they weren't properly listed on the application, so call the NY board. Also, if you plan to do an away elective in New York State, fill out this form waaaaaaaaaaaaaay in advance, of the away elective hospital won't let you join because they require this form on the first day.

    Update: I just remembered. You need a letter of good standing to be sent in by AUA in order for the Board of Medicine to process your application. I remember they put mine on hold for like three months because the application doesn't specify that you need this. The letter has to be faxed to:
