    Sharing or Discussion of USMLE Content Constitutes Irregular Behavior

    Examinees are advised in the USMLE Bulletin of Information that USMLE policies and procedures governing examinations have been established to ensure that no examinee or group of examinees receives unfair advantage on the examination, inadvertently or otherwise, thus jeopardizing the standard conditions and the principles on which the examinations are developed and scored. Provision of information relating to examination content by examinees may provide such advantage to individuals who have yet to take the examination. Examinees are directed to maintain the confidentiality of the Step examinations and are directed not to provide information relating to examination content that may give or attempt to give unfair advantage to individuals who may be taking the examination.
    The following is a statement from an examinee who disseminated content from a Step 1 examination and, as a result, was determined to have engaged in irregular behavior.
    To Whom It May Concern:

    As a student at American University of the Caribbean School of Medicine, I received excellent training and direction as a future physician. I was taught the importance of medicine academically, but failed to realize its importance professionally.

    After having taken my USMLE Step 1, I made a nearly career-ending mistake. I returned home, went through my study aid book, and noted from memory any question topics and specific test questions I could remember to help my friends prepare for their upcoming exams. Earlier that morning, I had signed a document stating I will not reproduce, by any means, secure USMLE content. Through my actions, I failed to uphold the integrity of our profession, not appreciating the importance of my signature to the USMLE pre-exam agreement.

    After having done so, I was called before the USMLE Committee on Irregular Behavior for a review of my case. I faced a lifetime ban from ever taking a USMLE exam again. I was fortunate enough to be banned for two years, however with permanent annotation of my USMLE transcript of Irregular Behavior, as well as notification to ECFMG and FSMB. I am genuinely grateful for the lesson I have learned. However, I wish I did not have to learn my lesson in this way. My transgression had the potential of undermining the validity of other examinees’ Step 1 scores and, on a broader scale, licensing and other decisions made based on those scores. While I thought I was simply helping my friends, I was actually doing them and our profession a disservice.

    I want to bring this to your attention, so as to emphasize adherence to the rules, with preservation of integrity and professionalism. I want to remind all AUC students to take USMLE’s rules regarding the integrity of its testing process seriously for failure to do so, as in my case, could have dire consequences on your future as a physician.

    I thought all of First Aid was written from past experiences of students who have taken the exam. In addition, while listening to Goljan's audio, he mentions many students who tells him what was on their exam.... I don't quite understand how First Aid gets updated every year if nobody is to talk about their exams.

    I'm curious how you would differentiate someone casually conversing, "oh yeah, I had a question on CAH, know that pathway" or "that picture was on my test," from what this person did? How did they amass enough evidence against this individual to ban him thus? Did he release a document of his findings or something?

    Oh well... back to studying...

    I received this email today as well, however I didn't go and post it on VMD.
    This is one of those things where u try to avoid other people finding out about it because it makes the school look absolutely terrible.

    I don't think the mistakes of one person make the whole school look bad. I've never understood that. Someone made a mistake...get over it. It's not like people don't discuss test questions all the time, all over the place.

    When there are school shootings, do you think the whole student body are a bunch of murderers?

    What's next....if we post on VMD that students should use usmleworld over kaplan because a lot of the questions were similar to the actual USMLE, do we have to grovel, beg, and write letters to become docs too.

    I think its too far to ask that a student can't highlight first aid or mark USMLEworld TQs after the step, and quite frankly, while I am compliant with the rules, I think its an overstrech.

    Its one thing if a student copies actual questions and then circulates them (which is copywrited material). It's quite another prevent a peer from recommending to another peer that they know a concept such as amphotericin B causes renal dysfunction (which is a medical concept that can't be copywrited).

    By demanding that students do not circulate copywrited material, and also by demanding that students do not discuss medical concepts that are not copywritable but that are tested on the exam, it's almost a 1st amendment infringement, and in my opinion, its difficult to argue that a student cant tell another student that they should be familiar with the side effects of important antimicrobial drugs or any medical concept for that matter. That to me violates the first amendment. (i know my 5th grade civics teacher said no right is absolute, but the first amendement was written specifically to protect people from govt. and other powerful entities misusing their power).

    In my opinion, trying to gag students this way and trying to instill fear that they cant become a doctor if they tell their best friend that ampho B side effects is an important medical concept to be familiar with is exactly what the First amendment was intended for - to protect free speech against misguided authority trying to spread fear and limit free speech, and while I respect the USMLE people, I put the constitution on a higher pedistal.

    This email really isn't just about irregular behavior....this email is more about spreading fear about saying anything about the concepts that are tested on, and trying to make people afraid about discussing or saying anything outside of "i took the test. I cant say anything else''
