I want my USMLE Step-1 application form to be sent for processing. I will fax my USMLE step-1 and step-2 score report to 708-235-1942 immediately. I understand that failing to fax the score report both front and back pages would delay my Official USMLE form to be processed by the school.

I also understand that failing the USMLE step-1 next time would make it mandatory to take a intensive MARATHON USMLE STEP-1 REVIEW ( 15 DAYS COURSE ) FOR RETAKING THE EXAM.

I also understand that I will be required to have a faculty advise on USMLE preparatory exam and my academics.

Mod, how can you fill out this form before you apply for your first attempt? Some, or should I say most, of these questions do not seem to apply to a first time applicant.

Whats Faulkon?

They want this in before you apply for Step 1. Also I hear there is a new 50$ to process the USMLE app.

$50 app fee...you gotta be kidding..this school is starting to become a major fleece operation with all these fees...

well personally, I applied from the ECFMG and sent in the print out to Pa office and they said it was being processed when I spoke to them on Friday...no mention of that sponsorship app or fee...I'm sure that it might be added onto my invoice along with other fees that I was unaware of.....the joy

I am not sure what this form is. I applied for the USMLE. I got a date as well as a confirmation form from ECFMG to bring with me when I take the exam.
