Pain : PQRST

P - position
Q - quality (sharp, dull)
R - radiation (groin, back, R or L shoulder, throat)
S - severity (on scale 1 to 10, does it wake you up at night)
T - time (any time of the day that pain is better or worse, related to meal,
bowel movement)

Dont forget the O=Onset!
Focus Hx should be OPQRST and FOAMASH

SAMPLE
S= Signs/Symptoms (associated)
A= Allergies
M= Medications (being taken)
P= Past Medical History
L= Last oral intake/ menses/ bowel movement/ incident
E= Events leading up to chief complaint

For Trauma:

DECAP-BTLS (As in decapitation as a trauma concept + BTLS)

Check for

D= Deformities
C= Contusions
A= Abrasions
P= Penetrations/ Punctures
B= Burns
T= Tenderness
L= Lacerations
S= Swelling

And for types of Shock its CRAMPS-NH (New Hampshire)

C= Cardiogenic Shock
R= Respiratory Shock
A= Anaphylactic Shock
M= Metabolic Shock (Diabetes, etc)
P= Psychogenic Shock
S= Septic Shock
N= Neurogenic Shock
H= Hypoglycemic Shock
