Review TOMMY concepts

Pharm…
A 30-year-old male named Joe PizzaHut presents to the emergency department complaining of a 4-month history of fatigue, depression, insomnia, and weight loss. He states he also has had difficulty with feelings of guilt over his IV heroin use. On a mini-mental status examination, he has some mild cognitive defects, scoring 22 of 30. What med is due most urgently?
1-SSRIs
2-Tricyclics
3-Methadone
4-First order U/A

should be given Methadone... used in those that are dependent on heroin and morphine; this drug does cause withdrawal symptoms but much milder than what they would get from heroin and morphine withdrawal, and does not last as long..

it is not clear if he stopped using heroin or he still does, because if he still does then change it with methadone ....if not.....think of something for depression
