That should give you most if not all of what you need to know about the Irish schools and applying to them.
As for a "transfer", I would not hold out much hope of gaining credit for your Charles studies and moving directly into 2nd. year in Ireland - but you've nothing to lose by trying.

510 points Officially, they do not accept transfer students.
If you apply through the normal routes and get in, you have a good chance of getting credit for exams you have finished in Prague.
A friend of mine took first yr in prague, applied to the RCSI normally, got a place on the basis of his leaving certificate results and went straight into second year.
So, if you qualify for a place, you can get credit for stuff you have finished but you will not qualify for a place simply because you get good grades in prague if you do not have the original entry requirements.
The problem is that while the exams in prague are of an equivalent standard and recognised in ireland, the entry requirements for international students are much lower in prague than in ireland.
