Answers for Questions in back of Blueprints for Surgery

I was hoping someone on this forum has the latest Blueprints for Surgery (4th edition). I have a copy of it, but the answers were torn out from the back. So if someone has it, it would be greatly appreciated if you would kindly reply with the answers (letter only is just fine, but all the better if you have the explanations).
