Greenbook requirements by State

I've been trying to find the easiest way to determine which of the 50 states require IMGs to do their clinical rotations (aka clerkships, preceptorships) "Green Book specialty" (vs. simply being under a family practice umbrella). For those not yet familiar with this situation, it means the hospital at which the medical student is doing the rotation has a residency program in that specialty (i.e., the hospital has residents there for a surgery, medicine, OB, peds or psych ACGME-approved residency).

Until yesterday, everyone with whom I spoke said I would have to call each state's medical licensing board to determine whether or not that state required the ACGME-approved rotations to be by specialty. Yesterday, a gentleman with the GME (graduate medical education) side of the AMA called me back with a list of states that, according to the FSMB (Federation of State Medical Boards), requires ACGME-approved rotations.

This only means that these states require the rotations to be "Green Book". It doesn't mean that they further require the rotations be Green Book by specialty. But, since this list is only 20 states, it significantly narrows the list of states that would need to be approached to ask about "by specialty" requirements.

Of course, there's a caveat. This list only includes the states that informed the AMA that they have these requirements. If the state didn't inform the AMA, that state isn't on the list as having a Green Book requirement. For example, I understand that TN has a “by specialty” requirement but the AMA doesn’t have them listed as even having a Green Book requirement.

If anyone has any more (or differing) information about the chart, please PM me so I can keep the list accurate for everyone’s sake.

Also, if a few people are willing to call a few states’ medical licensing board in which they are interested in practicing, we can cover all 50 in no time without any one person having to spend much time doing it.

Thank you very much for the list. Can you just help me clarify something: if there is a cross in the column saying "Requires ACGME Approved Clerkships", does that mean those states will accept rotations done under the family medicine umbrella or they all must be green book by specialty, i.e. have the residency in those hospitals? Thanks a lot again for your help.

I was afraid I didn't explain well enough. The X in that column just means that the AMA was informed by that state that the state requires ACGME-approved rotations. That's what I'm hoping others on the site can tell us:
1. Which of those states require just Green Book
2. Which states require Green Book by specialty
3. Which states require either #1 or #2 but aren't listed on here (because the AMA hasn't been informed).

Again, as previously stated:

Contact the respective state board!!!

Always remember, that each application is done on a case by case basis. Have fun in rotations...

States Requiring Green Book Rotations by Specialty

If you have researched any of the states and know whether or not they require rotations to be not only Green Book (ACGME approved) but also "Green Book by specialty", please PM me with the state you have contacted and from whom you got the information.

I am in the same boat as you are. According to my knowledge, there are two states, one is TX and I forgot the name of the other one. These two states are the ONLY one that requires 'greenbook by specialty' clinicals. Everybody else, I am assuming requires 'greenbook' under the family practice umbrella.

As a side note, I have been told 'greenbook by specialty' clinicals are more intense and ofcourse more beneficial in terms of experience.

so that's what you mean by "green by specialty"....... green, but not under an umbrella.

another thing to note....... Nevada also requires green electives.


i don't know which other states req green electives too.

Right. The "by specialty" means that the state requires that the cores to be done at a teaching facility that has a NCGME-approved residency in each specialty, not just a facility that has has an approved Family Practice program. If there is a more technical term for it, I'm unfamiliar with it.

Thanks for the note on NV. I had no idea. I thought I was out of the woods after cores....

I don't think I would want to do rotations (cores or electives) at a location that was not Green Book but hopefully no states require that the electives are Green Book by specialty.
