Texas and rotations for license

So, if I've completed a few electives at hospitals that do not have a residency in that subject(such as radiology), will this be a problem down the road for Texas licensure?

I've heard that getting board certified BEFORE applying for licensure can bypass this obstacle.

Thanks!

I know that TX and PA like all rotations to be green. Do you know which states require cores to be Green (rather than AOA)?

I'm at the point now where I have to decide between moving around between green rotations or staying in one spot and doing AOA.

though this might be true for TX, i know for sure that as of 2006, PA has changed it's licensing laws. they require 3 years of ACGME residency; they don't require green book rotations anymore. i had the guy from the licensing office say it twice to me b/c i didn't believe it.

Thats funny! I spoke to a guy there too today and was told that rotations had to be green. I guess conflicting stories do abound!

Who was the guy you spoke to? My guy seemed like an older gentlemen, soft spoken and willing to answer all of my questions.

he was a younger guy, and i don't know his name. he basically said that IMGs were having such a difficult time getting their permanent license in PA that the state medical board had a meeting and determined that 3 years of ACGME approved residency would satisfy their requirements instead of going through all their rotations. he also said that they accept FCVS now, which they did not before. I spoke to this guy last year in Sept or Oct.

my guess (after this conversation) was that PA is suffering from so many docs leaving, because of the malpractice crisis over there, that they are making it easier for docs to practice in PA.

supply and demand, strong forces.

I guess there still isnt alot of consensus as to what the rules are. It seems like the rules change dependent on who you talk to in the Medical Board.

In any case, I totally hear you about docs leaving PA. That Scranton ordeal was a monster.

But what I cant figure out is why there still is the ban on foreign medical student rotations in the State. It seems to me from talking to many of the PA residency programs, that they would love to take FMGs if they were willing to stay on board and practice in the area.

As you probably already know, the trend is clear...FMGs that rotate in-house at hospitals have a better chance of matching there...which in turn translates to practicing in the area since a good portion of new residency grads practice within a 200 mile radius of where they did residency training.

yep, that's the idea that the states have. to fund a residency slot costs around $150,000 per year, or so i've heard (and we only see $40,000 of it). it's an investment for the state, and they hope to do good on their investment by keeping you there.

about rotations, i'm not too sure about that. but there are some IMG friendly residency slots. here's the site.

We prefer IMG’s to have had one-year clinical experience in the United States or the United Kingdom. There is no absolute cutoff for year of graduation from medical school. The decision to grant an interview is based on your clinical experience since graduation
SCORES: While we prefer USMLE scores (Step 1 and 2) of 200 or greater, there is no absolute cutoff. Each application through ERAS is reviewed individually and a decision to grant an interview is based on the applicant’s overall qualities.
Again, let me emphasize that there are no absolute cutoffs to eliminate applicants from the selection process. Each application through ERAS is reviewed individually and in-depth. Decisions for offering an interview at Pennsylvania Hospital are based on the applicant’s overall strengths and qualities. Also please have every effort to have your ECFMG paperwork in working when applying and completed by the time the match list comes out in mid-February.

Above is an example of what the FSMB stated in its report that licensure in the USA is the toughest ever in its history, imagine that most programs requires scores of 80 or above on USMLE steps and take only recent grads, and many take only first attempt successful candidates and repeaters ineligible to even apply,!!!that's only one step in one's career, being successful in residency is another challenge and I have seen many kicked out of residencies for many reasons, applying for licensure is another big challenge, passing speciality board exams another big one[I sit on credential committe and in 2006 we asked one physician to withdraw privilges because unable to pass board exam and most hospitals now requires board certification 5 years after residency completion and 2 years post expiration of certificate otherwise they want you to leave,my hospital requires ALL physicians to be board certified and that eliminate any incompetency issues].
I am trying to make a point that when "the extreme few" try to state that a certian physician with all exams, residency and boarded is unsafe,I have to say "you just get to be kidding"!!!!& thank god that the extreme few are buying that.
