hey everyone, i know this is the may 2008 acceptance thread but i thought it would be one of my best options to ask you guys. the health forms... with the copy of the test results, how many do i need, 3 or just 1 for the Dominican government?

also what did you guys do with the media consent form?

You need three forms of each kind, and you are supposed to send everything to the jersey office, this is a new thing. I think prior students had to carry copies w/ them and get the visa once they got down there, but now the jersey office is handling the visa distribution so they get everything. However, just to be on the safe side, i've made one packet worth of copies of everything i've submitted to the jersey office. Call your student coordinator to find out what exactly you're missing.

Yes, a copy of each of test results. And, regardless of your ppd being negative, you MUST submit a note from the doctor indicating that the test was done and what the reading was. They gave me havok about this one, even though my ppd test results were negative.

You should've been assigned a student coordinator, call the Jersey office and talk to them asap!

Don't forget you need a chest x-*** even if your PPD test was negative!

change of subject...

I will be arriving on the island on May 3rd, but my family member will be arriving on May 4th. Do I have to arrange for someone to pick her up? How will this work?
