Just to get the fact straight - you don't "lose a year" if you start derm at PGY-2.... I guess you are still very early in the game to talk about residency aren't you.

Programs like derm or anesth. do their PGY-1 year in general medicine (AKA the transitional year) and then spend the next 3 years in the actual field. So PGY-1 derm positions are the same - they still have to do general med first. This is also why Psych is 4 years.

I was just wondering:
To go into derm or anesthesiology or psych, does one just match into a transitional year program, or into a derm or anest. or psych program?

Almost all psych programs are integrated (eg four years without having to switch institutions), though there are a handful of programs that start PGY2 or have open PGY2 slots. In that case, one has to do a medical transitional year (see page 14 of http://www.abpn.com/Downloads/2005part1_ifa.pdf ), while entering the match for a second year in a row. Stats from last year's match show 1,020 PGY1 positions and 5 PGY2 positions. Obviously, some additional PGY2s were filled outside the match.

Most gas programs are three years and require a transitional year, called a "clinical base year" see http://www.abanes.org/booklet/BOI-2004.pdf section 2.02, although some programs are integrated and therefore four years in length. 443 PGY1s were offered in the 2004 match, 322 of those were filled by US Seniors; 846 PGY2s were offered, 587 of those were US Seniors.

Ditto for dermatology, though they have their own requirements for the transitional year, see http://www.abderm.org/docs/pdf/BookOfInfo.pdf
As far as the match goes, 31 PGY1s were offered, 30 were filled by US Seniors; 263 PGY2s were offered 208 of these were filled by US Seniors.

question

Ok, I am a little confused and new at this game...Which IMedical Schools can get the best residencys for their students and how do you go about getting a residency? I am a US native and interested in going to a IMedical School. Is Dermatology and Surgery totally out of the question for IMedical School Grads? Which hospitals like IMG's? How hard are the boards? Which is the most reasonably priced IMedical School, that is a great school? What about getting licensed in Texas? Thanks

That's a lot of questions. I will only answer in part.

Generally speaking, if you go abroad, you will not be as competitive as US grads. It is for this reason that the most popular (and financially rewarding) specialties (see previous page for stats) are populated by US grads.

As far as Dermatology goes, if one goes abroad, one can in all likelyhood forget about it. It is that competitive. Many US students with top qualifications do not match into that specialty.

Surgery has been discussed multiple times on this forum, search the forum with keyword surgery.
