actually,
this is how it works. they send out requests to US faculty all over the country. the ones that want to write for USMLE go to Penn for an intensive question writing boot camp. then they submit questions, which over the course of 2 years is modified and passed through several committees. THEN you see it on the exam. over various stages of its development, the question is put into the experimental portion of USMLEs.

this info was told to me by not one, but two writers for the step1.

yeah its nuts. also, you aren't like given an avg according to who you take the exam with (i.e. - if you take it w/ US students the mean isn't higher to pass). its crazy how it all works, i have a much higher respect for the exam now.
