I think the internet is the biggest culprit, because of the world wide web, countries without modern infrastructure can do business. Bricks and mortar aren't as crucial as it was in the past. A good amount of my medical education is done over the computer, at my medical school I am expected to finish assignments over the web and email them to my professors.
Some guys find Asian women fascinating, true some of them are cute. But a lot of Asian guys I know daydream about an American/Euro/Aussie blonde. A Korean dude a few days ago was staring at some platinum blonde and rear ended me in a fender bender(I was staring too )

There is a strange juxtaposition of extreme wealth and poverty in America, especially in the larger cities. I just came back from a mini vacation in Miami, it was jarring to see poor slums next to neighborhoods with Ferraris and 20 million dollar mansions. This isn't the only city to have this level of polarization in America. I have seen it in Chicago, NYC, LA, SF, the list can go on. What does this say about Americans when fellow Americans don't have enough to eat, a bed to sleep in???? You don't see this wide disparity in other developed countries like in Canada, Australia, New Zealand, Japan, Singapore, and Western Europe.

Did you see the movie the Producers??? There was a line - "When you got it flaunt it"... I would expect to see the rich get richer and the poor to have children (F Scott Fizgerald - the great Gatsby)....well into the 21st century

Most of us future physicians will probably be at the top of end of this system. Its not uncommon for even primary care attendings to earn in the low 200K range. Still I don't think being rich when others are struggling would make me feel accomplished and happy. In many ways I might feel ostracized. Its still going to be a wake up call for the US when many countries around the world are experiencing an increasing number of middle class while the number in the US is decreasing. The biggest reason why the US remained a solid democracy for such a long time was a strong middle class. In the beginning the of the last century the USA had a fairly equitable distribution of income while Europe was highly unequal, that has reversed with Western Europe being more equal and the US being more unequal.
I would not be surprised if the US in 2030 looks like the police state in the Running Man movie. This is assuming that nuclear armageddon does not happen. Looking at things right now, two additional nuclear powers, North Korea and Iran, a few more down the road including Australia and Japan, maybe even South Korea the end of days isn't too far away.
