In his room, he begins screaming for his nurse, stating that there are people out in his room trying to get him. He gets out of bed and begins pulling his IV line. On exam, he alters between agitation and somnolence. He is not oriented to time or place. His pulse = 126, resp = 32, BP = 80/58, temp = 102.5. What is his DX?
