old man Q

66 yo african americn male is being hospitalized for acute pneumonia 3 days previously. In his room, he begins screaming for his nurse, stating that there are people out in his room trying to get him. He gets out of bed and begins pulling his IV line. On exam, he alters between agitation and somnolence. He is not oriented to time or place. His pulse = 126, resp = 32, BP = 80/58, temp = 102.5. What is his DX?

a. dementia
b. schizophreniform d/o
c. schizoaffective d/o
d. fugue state
e. delirium
f. brief psychotic d/o
g. schizophrenia

I'm not too sure, but since he is not oriented to person or place, delirium sounds right...

right! delusions and disorganized thinking suggest psychotic d/o but fluctuations in consciousness confirm delirium
