A 90-year-old, non-ambulatory male with decisional capacity is ready for discharge from a rehabilitation unit after a left above-knee amputation. He may now be unable to safely exit his second floor apartment to the ground floor in case of an emergency. The patient wishes to be discharged back to this second floor apartment that has no elevator. You should

(A) encourage the patient to move to a safer home
(B) contact adult protective services to manage the discharge
(C) discharge the patient home without any further discussion
(D) discharge the patient to a nursing facility after obtaining a court order
(E) not allow the patient to be discharged to this unsafe situation

I would d/c the pt to a nursing facility since hospitalization is no longer appropriate and his living situation is potentially unsafe. This pt is 90 y.o. and will require some assistance with caring for himself as a new amputee, so this would be most appropriate.
