I would make sure that the particular government of the Island you choose will allow you to enter based on the DUI. I would also make sure you can do clinicals, and residency in Canada. I know that Canada will not allow American citizens to enter if they have a DUI on their record unless you get special permisson for a very limited stay.

As for doing clinicals and residency in the US you would probably have to check with Homeland Security, and or The State Department and explain your situation. As I see it your main problem would be the US federal government. I have it from a reliable source that Americans going to Canada with a past DUI (more than 10 years prior) still have a VERY difficult time and are turned away at the border.
