practicing in costa rica

I am interested in going to a caribbean med school. I plan on practicing in costa rica, nicaragua, panama area. I have a 3.2 and no MCAT. What am I looking for since I am not going to practice in the US? Will it matter which school I go to as much? Anything to stay away from?
