It's easy to think that schools will forgive mistakes made in the past and focus more on recent achievements, but this is unfortunately not the case. You have to take into consideration that while you may do well now, there are far more qualified students than spots that did well all through college. You had better get some pretty amazing MCAT scores if you want to go anywhere half decent. Honestly, your clinical / volunteer experience is ok, but certainly nothing special.
Don't waste your money applying to the US (where you have approximately a 0% chance). If you get very good MCATs and bring your grades up, you'd be lucky to get into AUC, SGU, or Ross. Definitely apply to SMU, AUA, and some of the other less-competitive caribbean schools, they'd probably be your best shot.
Sorry if this post sounds negative, but just be realistic, your past is going to haunt you. I have a 3.8 GPA for the past 2.5 years and a 27 MCAT, but old grades brought my cumulative down to a 3.3 (a little less when I turned in my AMCAS application). I got into AUC without any trouble, but I didn't get a single interview in the US.

When I applied to AUA - the first 25 minutes of my interview were taken up with my so-so grades at one of the schools I went to. Only when I pointed out I had a 3.55 in my MS program and a 3.2 in science did the interviewer see "I was a Serious Student". Your poor grades will dog you till the day you die.
