Post this on the Australian forum, however, MBBS and MD have been discussed on many occasions, no real difference apart from the letters, both are primary medical qualification.
Personal opinion, quality of education is better at the australian universities, and you would probably not be looked up-on as 'just another carrib. Grad'. however for returning to practice in the US, probably a carrib. programme would be better for you, allowing strong contacts to be made in the US. Good luck,
