L6 Form for California Medical License

Attached is the California Medical License application- the L6 form is on page 22. The L6 form needs to be completed if you are planning to apply for a California medical license. This form needs to be completed by each of your core rotation sites. Once the core rotation site completes the form, have the site give the form back to you so that you can submit it to the medical board whenever you submit your medical license application.
