Sree or anyone : need a very urgent help

Hi,

My clinical rotation is going to start around 23rd of this month.As I'm new to US Could you please tell me where can I get the following hospital required documents?

1) Local Police/Sheriff’s Background Report within one year
2) Proof of certificate of training for OSHA-Blood borne pathogens

3) HIPAA

4)Health Assessment (must be completed within one year of start date)
5)PPD Status
6)Titers (lgG) for measles, mumps, rubella and varicella (vaccination records are not sufficient) (MUST HAVE FULL LAB REPORT: NOT JUST DATES)
7)Titers for Hepatitis B, or proof of vaccination (series)

Please guide me.Thank you.

Let me get the first one......
Go to 1 police plaza in Manhattan(behind city hall) with a $30 money order or certified bank check(NO EXCEPTION) , and ask for certification of good standing .
They'll figerprint you and the letter will be mailed to you in 7-15 business day.Or ask them to hold the document, and pick it up, if you want it earlier.But speak to the ladies there and let them know that you will come to pick up before you leave.Be extra couteous to them, ask them if they want coffee etc, they can get it done in 3-4 days.

Take all your ids with you, they wil check you like you just came off the boat, when you enter the building, so relax, thats typical NYPD style.

BTW, its a beautiful and historical place.I took a sandwich and nibbled on it once I came out.Bought a OFFICIAL NYPD cap, from the NYPD gift shop.

Take the subway, parking is difficult and expensive.

For the health requirement you can go to a urgent care, they are usually walk in and they do physicals, ppd, vaccines, titers, basicaly anything u need.

1. your local police department
2 and 3 were done in Miami for me during the first week
4, 5, 6 - if you dont have a doctor, go to an urgent care clinic - some of this stuff might be expensive if you dont have american health insurance though

Hey Sree,
For 2 and 3, you said we did them in Miami. Did we receive a certificate of some sort? If so, can you describe it. All I have from Miami is the ACLS card.
