AUA does have a radiology elective (at St. Joe's in Michigan). We also have Orthopedics at Union Memorial. I believe those are the only two non-primarycare residency programs that AUA is affiliated with.
It is not easy to set up your own rotation because bigger schools like St. Georges have exclusive access or preferred access to certain places. Your step I score and transcript may play a role. FMG friendly hospitals are probably the only places that will take you - but I know that Downstate Medical school has spots for foreigners as well. The hospital gets some of your tuition, I assume. The application process isn't too bad.

So I'd have a really tough time setting up a rotation at a Massachusetts/Boston area hospital?

I'm going to send AUA a letter requesting away rotations sometime this week and post here, how it goes.

at mcmaster university (canada) to request to take an elective there as an international med student you need to apply 6 months in advance, submit a fee and etc, and must be in ur last year of clerkship
i know some IMGs who did electives at mcmaster so it is doable check it out |

I've been busy, but a friend of mine who is in Surgery with me just applied for an away rotation in Neurosurgery at Henry Ford and he told me there was a 3 month delay but he ultimately got approval from the school.
