Medical School process in Pakistan for foreign student: HELP!

I am a second year premedical student in US and I want to apply for Medical school in Pakistan. I have dual nationality and I want to apply through the Pakistan Technical Assistance Program. I have the equivalence form and the application for PTAP but I am confused about the process and I also do not know the due dates for these forms. I would really appreciate someone's help asap.
