Hot off the press!!! Here were my cases:
1. Depression/constipation in young woman. Ordered labs, she was hypothyroid! Synthroid given and she improved.
2. Asthma exacerbation s/p URI - straightforward.
3. Urosepsis in a guy s/p TURP.
4. Acute MI presenting as nausea and vomiting in a diabetic patient. Rather subtle presentation. EKG showed STEMI, labs showed glucose 400. Do not get fooled by treating DKA. Give a shot of insulin and take for cath.
5. Pre-procedure eval for a young woman with a breast lump. She mentions she has easy bruising and has petechiae all over. I ordered CBC and plasma antiplatelet antibody, which came back positive. Dx: ITP. Treat with prednisone.
6. Chlamydia urethritis in a young male - simple.
7. Female with cough, nightsweats, chills, esophageal thrush. Works as construction worker, multiple sexual partners (read SH carefully). Guess what, turned out to be HIV positive with PCP pneumonia!
8. Young girl found unresponsive in a restaurant by friend. History states "she took some pain pills". On labs, glucose was 23. I gave glucose and Narcan, she responded. Became lethargic again, Narcan given, responded. Patient then stated she took gluburide 15 pills. I think she took that and opiates too. She was totally suicidal - I put her on precautions.
9. 14 year old girl with fever. On exam found to have tampon. I treated her for TSS. She improved.
