Will the caribbean school call the people that recommended me for a followup. This might be a problem because 2 of the people that recommended me are currently on vacation. Will this effect my chances of admittance if the schools cannot get in contact with my recommenders?

I don't think they typically call the references - they just read their letters. No school can expect that all faculty will just be sitting their waiting for medical schools to call and check up on references.

I'd suggest you get letters from the attendings you work with at the hospitals in the US. It won't be a problem if you use them, but it just looks better to get it from Program Directors at the US hospitals.
