Is it better to start in May or September ? are there any real advantages to one over the other...?

I would prefer May semester just because you are ahead of the game by 4 months compared to those who start at the traditional time in September. If anything unexpected occurs you will still be able to match at the same time as the August class.

The sooner you start the sooner you can hang up your MD shingle. The most important item is scheduling everything so you don't have to wait an extra year for the residency match.
