View Full Version : Please help -Q on Goodpasture's Syndrome

A patient presents with symptoms and signs of Goodpasture's Syndrome( :wink: with lots of bla blas suggesting the disease)
The mechanism producing this disease is closest to the mechanism underlying which of the following disease?
a)Bullous pemphigoid
b)Grave's disease
c)Hereditary angioderma
d)Rheumatoid arthritis
e)Serum sickness

From Kap lan source the answer is a)Bullous pemphigoid, of which there is explanation in which at the end concluded that both has Type II Hypersensitivity Reaction.
:? Now my concern is , b)Grave's disease also has Type II Hypersensitivity Rxn.
Can somebody please explain?

I would guess it is because in bullous pemphigoid
there are antibodies -IgG- and complement (c3)
directed against the basement membrane in mucosas
and in Goodpasture too there are ab directed to the
basement membranes but of lungs and glomerulus.
This ab attack causes destruction of the basement membranes.

In contrast in Graves disease the thyroid gland is
under continuous stimulation by circulating autoantibodies
against the TSH receptor.
The stimulating activity of TSH receptor antibodies
is found mostly in the immunoglobulin G1 (IgG1) subclass.
THis ab cause overstimulation and not destruction
of the thyroid gland.

Those are the diferences i found...

Another answer choice here could have been systemic lupus erythematosus where Abs can be directed againts the basement membrane of lungs and/or kidney glomeruli.
