Hello, I am a 3rd year and did a 2 week elective in the SICU. Do I need to get some evaluation form filled out by the attending in charge or resident or does the school take care of all the paper work? Someone told me that I need to go to the secretary to get an evaluation form and then get it filled by the attending. Any info is appreciated.

you need to get an evaluation form filled out, go to the clinical coordinator at the hospital you did it at and get the form (1 page, usually blue) and get it signed by the primary attending you did your work with. Wasn't this explained to you on your first day of starting the elective?? The hospital coordinator should have told you about this stuff. You should also familiarize yourself with the ClinEd website to help out with any other things in the future.....

lol, yeah, I sound like I was in a bad mood, eh? It was really more aimed at the clinical coordinator at the hospital though, this stuff should be mentioned to noobie students on their first rotations/electives, after all, the coordinators have usually been doing this for a while and I understand how it can be confusing for a student (especially if there are multiple schools rotating at the same site). The inept people at the school are getting to me, I'm heading down to MCH for my peds elective and I literally had to ask the housing lady 5 times for the number of the person to contact to let me into the school housing when I get down there :rolleyes:

But, also onus on the students....seek and ye shall find, ask a classmate or heaven forbid ask the coordinator what you need to do to obtain credit for your rotations....

didn't mean to call all the ruckus! i was never told of any evaluation form or given one. I guess I will go back to make sure its filled so I can get credit. I hate the way the school expects you to figure out things.
