Yes having citizenship does help, without it, you will need to be sponsored for visas, that makes it harder for you. You will have it tougher looking for residency programs that will take you.
Also, being a citizen or not, as a foreign graduated doctor you will be looked on less favourably than home grads in N america.

why not practise in taiwan? it is not bad, is it?
if i were you i would be happy to serve patients in taiwan. as a doctor, we shouldn't choose patients based on their country - we couldn't say, we only treat ppl from wealthy families/countries. most probably good doctors are in need much more in undeveloped parts. yes, income is an important element when many ppl decide to study medicine however i could not choose to be born in a perfectly developed country and money should not be the first concern when being a doctor.
finally as a gp or specialist in taiwan you also receive satisfactory income. it's really not bad.
good luck for your N american citizenship application as well as your medical career.

Citizenship is not obtained just by snapping your fingers. There is a process to it and different ways of obtaining it. If you are resourceful, you can access the immigration website of either country and read, educate yourself on how to obtain citizenship.
In general, you must first be able to enter a country legally either with a working or immigrant visa. There is usually a time element involved before obtaining residency status or citizenship. Just read up on it. Also, there is usually no need to have a lawyer do this for you as the process is simple.
