It hasn't. Even the faculty, with 2 exceptions, aren't vetted to practice on the island.

With all due respect, I would direct this question to Saba University administration itself, rather than on this highly informal forum. Most of us, including myself, are students and do not know nearly enough to give a complete answer to your question. I don't personally know any Saba students with a license to practice on Saba or in the Netherlands-Antilles, but my circle is of course relatively small.
However, are you asking this question because states like Texas will likely not consider Saba equivalent to their state medical schools unless Saba students are eligible to be licensed in the Netherlands-Antilles?
Is the intended question should be "Are Saba students eligible to apply for licensure on Saba or in the Netherlands-Antilles?" The answer to that question would be yes, we can, just like Ross students can apply for licensure on Dominica.
