California L5 and L6 forms

Couple question, hopefully someone can help me out with these. (1) Are we required to submit L6 for all sites, some sites, or no sites? (2) Should I complete L5 as I complete each rotation? Thanks in advance.

the L6 needs to be done for each rotation you do.

where do we send our L6 Forms?

How does ERAS work? Do we open an account at the very begginning of clinicals and have letters of rec directed there or to MEAS?
