Hi all, just the first week in internal med. Wanted to know how do we do our weekly case 'write-up',do we have to write out history and physicals and progress notes or just H and P's ? any tips or ideas would help.

I would assume it would depend on the hospital and who is grading it, but for us it was:

1) H+P
2) Problem list/A+P for each problem
3) Short 1-2pg discussion of the disease entity and its management.

I would think a full note is required as EMT said (with labs, A&P, etc) but probably with emphasis on a good history writeup. At Methodist, the emphasis was def on the history with inclusion of pertinent positives AND negatives.
