i heard somewhere that if you start your 3rd year core rotations (after having taken the Step, but not yet having received your score), this makes you ineligible to do 4th year electives at unaffiliated hospitals or something like that?

is this true, can someone provide some clarity/more information?

Yes, that is true for the state of New York. If you begin clinicals prior to receiving a passing Step 1 score then you are not allowed to do unaffiliated electives in the state of New York.

this is a very limited thing, but complicated, but i will try to explain.

If you are doing rotations in NJ this does NOT pertain to you... to do any rotations as a medical student you must, per NJ state law, have a PASSED step I score to start, so....if you are assigned to a NJ hospital, you can stop reading now....

and as far as i know, if you are doing cores in any state other than NY, again, you probably can stop reading too, though i am not 100% about this, so double check this...

now to NY...in NY you have to fill out paperwork to be able to "work" as a medical student in NYS. There are 2 types of applications...one which is a full certification (for lack of a better word- can't remember what its actually called) and this REQUIRES a passed step I score. So if you fill out this app, then you have no worries...there are no limitations to this application.

the 2nd one is a limited certification to work as a med student...this app allows you to apply with a PENDING (important here, you have to have TAKEN the thing...) Step I score. So the advantage for this is of course that you can take step I as late as possible and still start on time (no waiting for scores to report).

BUT, and here is the BUT.....

NYS will only allow 12 weeks of unaffliated rotations (why sgu only allows 12 weeks) within NYS (key here...if they are done in another state, they don't count , in NY's eyes, as unaffliated)...this is for both forms of applications,


and now the BUT!!!

IF you apply under the limited app, then NYS will count the weeks you do before your scores report as "unaffliated" in their eyes..and its an all or none deal(because basically you can do 12 weeks of med or surg during this time) and like you lose your grace period when you consolidate your loans, IF you do the limited app, then you use ALL of your 12 week allotment for unaffliated rotations...AND the kicker?? once you file the limited app, you are stuck with it...even if your scores come the very NEXT DAY, you will always work in NY with the limited application.

now here is where the limitation really is...this only pertains to people who are planning on getting a license (training or unrestricted) in NYS...if you get a residency inany of the other 49 states or will practice in any of those other states...you can stop reading...in fact you could have stopped reading a while ago....


SO......

IF you are planning on practicing in NYS or
IF you are planning on doing your residency in NYS,

AND, if you work in NYS as a med student under a limited application, you CANNOT do ANY rotations in any other hospitals other than the ones affliated with sgu IN THE STATE OF NY.

So, where does this come into play?? So if you want to do an audtion rotation at an unaffliated hospital in NYS (because you wanna do your residency there), you CANNOT count it as part of your requirements for getting your MD...you will more than likely need to do additional rotations to make up for the weeks(and at 300 extra a week...) otherwise you won't meet the requirements for NYS.

long winded i know, but hopefully it clears somethings up.

Very well put, Roks. I was in this situation (took Step 1 scant two weeks before 3rd yr start date) and unable to do any unaffiliateds in NY state, though the rest of the country was wide open.

Unless you're looking specifically for an audition rotation, there's varied enough electives offered within SGU affiliates to cover nooks & crannies of pretty much any specialty's sub-sub-specialty. Also, some universities deliberately make the process so tedious that by the time one wades thru all of SGU red tape & the hospitals spec's (not to mention the expense), one might decide to just not bother.
