hi everyone. Is there anybody out there who has already completed the health certificate portion of the application? If so, what exactly do they want? my physician said it was very vague. general blood and urine tests? mental health? chest X-***? He said unless it's more specific, this whole check up could be very expensive. Any help would be great. thanks. I really don't understand why this is part of our application.

I have completed the health certificate. My current health insurance allows an annual physical exam with blood and urine tests included. I submitted a report from a chest x-*** that was performed several months ago. The mental evaluation was part of the physical exam. Because all of these tests were unremarkable, the physician simply wrote "normal" on the line beside each section on the form.

I do not know what the cost for these services would have been, but I didn't pay anything.
