I am planning on sending in my police report, medical report, and proof of roundtrip ticket to the New Jersey office sometime next week. I was wondering if Ross will be able to issue me the visa on time. BTW, I will be starting in may.

Don't worry, nobody gets their VISA before they leave for Dominica. You will enter Dominica on a one-month visitor visa and should get your student VISA after you arrive.

Thanks for your reply Mitch. So you are saying that no one will ask me for any documentation when I arrive at the Dominica airport??

Also, I have another question. I got all the medical tests done (HIV, PPD, Chest X-ray, physical) and they all came out negative. Is it still mandatory that I submit my chest xray imaging report or do they let me get away without turning it in......'cause I've to make another trip to the doctor's office for that stupid report.

You should bring a copy of your acceptance letter to Dominica with you. They ask some students about it and don't for others. Either way, they will let you through, but it may be easier with your passport and acceptance letter.

While your exams may be negative, there is no way you can prove that to the government if they don't have a copy of your report. With that said, I would call the office and ask. They didn't require the x-ray when I applied unless your TB test was postive (current or past).

You need to have the X-ray imaging report, weather it's a positive or negative TB result. Please keep in mind that the medical documents for the visa has to be fill out by hands for all three copies.( No photo copies are allowed). Along with that you need to have a letter of good conducts, international money orders (the pink one), and 2 passports pictures. I hope this is help though!!!

i did all the tests also, but i don't know my blood type and Rh. do i have to visit my doctor again for that? :?

Do we have to have three copies of the Imaging report (one for each health certificate)? I will be forwarding this question to the Admissions office, but if anyone knows please post the answer. Thanks.

For the "imaging report" my doctor just wrote something on the line next to where it said "Imaging report".
I think it said: Normal CXR
That's it. He said that's all I needed, that there wasn't some special xray report.

Did you guys get some sort of xray report?

Hi. To answer your question, yes you need a separate imaging report. I am assuming that your doctor did not perform the Chest X-ray himself and instead gave you a referral to a lab to get that done. (That's how it was for me.) In any case, wherever you got the actual Chest X-ray done, that doctor/ lab is responsible to give you a printed report, which indicates the result of the chest X-ray. That is the Imaging report that you need to include with your Health Certificate. You can attach one copy to each of the three Health Certificates if you want to. Otherwise, the Admissions Office will make the copies themselves. (Incidentally, for any New Yorkers, I got my Chest X-ray done at “Doshi Diagnostic” without an appointment but with a referral and it took only a few days to get the results.)

Actually the xray was done in his office. They took it, developed it in about 5 min. Then he put it up on the light, looked at it, and wrote on the sheet.
I asked about the report. He said that what he wrote is all I get.

I had my chest x ray report written on a prescription pad....It need not be official. I note on a prescription pad is just as good as formal letter head.

Hi. If that is the case, then I think that what your doctor wrote should be sufficient. Call the Admissions Office tomorrow to make sure that is acceptable and please post the reply here so it could help others. (Sorry, if I mislead anyone with my experience.)

Hi Everybody!
My doctor got my blood work back for medical certificates. Everything is fine. However, I do not have an immunity to Hep A (never been vaccinated, and T.G. never had one). He left that line blank, and suggested that I get vaccinated for traveling to the 3 rd world country. But his office does not even carry Hep A vaccines, so I may have to go to my daughter's peds doc to get it.
My question, though, Do I really need one for the visa, and more importanly for living in Dominica?

Also for those of you asking about CXR immaging reports. I had my done in the radiology diagnostic center (HMO stuff), and they faxed a copy of the report to my doc, and I'm submitting it together with my actual blood work, and medical certificates.

You can go to your local public health office to get it. OR you can get it in Dominica if you want. Hep A is an acute fecal oral infection from a picovirus (sorry, studying for boards) and I've never heard of anybody here getting it. Never hurts to have though.

I thought everything on the physical document was required. Anyway, I got my Hep A immunization shot for $32 at the local Georgia Public Health Department center last Thursday.

As for the imaging report, my campus physician filled in the blank on the physical document as well as an actual report on a separate paper.
